미니 스크루의 한계를 뛰어넘어 치아 이동량을 획기적으로 증가시키는 교정용 플레이트가 화제를 모으고 있다.

쓰리디케이피는 약 6년 여의 연구개발 기간을 거쳐 ‘3 Dimensional Key Plate(3DKP)’를 선보였다.
‘3DKP’는 구강 내 골의 밀도가 가장 높은 구개골에 간단하고 효과적인 방법으로 직접 식립할 수 있는 치과 교정용 anchorage로 ‘절대 고정원’의 역할을 한다.
특히 구치부 치아를 후방 이동시키는 동안 통상적으로 발생하는 전치부의 Flaring을 만들지 않으면서도 단기간에 제1·2 대구치를 동시에 필요한 만큼 이동시킬 수 있어 교정치료의 효율성을 극대화한다.
또한 기존 미니 스크루는 숙련된 임상가의 경우에도 15%의 높은 실패율을 나타내는 등 한계가 있는데 반해, ‘3DKP’는 세 개의 돌기를 가진 플레이트를 각화치은으로 덮인 Palate에 절개 없이 쉽게 식립할 수 있어, 원하는 교정력을 Center of Resistant 높이에 적용할 수 있다는 장점을 어필하며 호응을 얻고 있다.
쓰리디케이피 관계자는 “‘3DKP’는 식립 시 절개나 드릴링이 필요 없이 Palatal Mucosa를 파고 들어가는 안정적인 구조를 갖고 있다”면서 “대부분의 Brachycephalic 케이스에서 비발치 교정 후 안모 변화로 Lower Anterior Face가 길어지며, 비대칭 케이스 및 무턱 교정 치료가 가능하다”고 밝혔다.

Conventionally, orthodontic treatment that aims to improve short LAF has been focused on inducing passive eruption of the posterior teeth using anterior bite plate. However such treatment method doesn't overcome the strong forces exerted by masticatory muscles thereby diminishing the success of treatment.
I report a method that involves installation of a palatal plate which can be easily utilized to treat these cases through support of over 20 years of clinical experience and expertise of colleague scholars.
It is an absolute anchorage called 3DKP that distalizes the maxillary posterior dentition only as much as it is needed.
WAVE1.2 is rigidly fixated by solder to the left and right first molar bands with 1.2mm orthodontic wire. In the case of Deep Cl.II, insert a .019 x .025 Curved Nitinol wire into the slot up to the first molar since the left and right first molars are fixed as one mass.
While the anterior inclination control can be expressed, the posterior segment from canine to second molar can be distalized simultaneously.
You can insert adjusted .018 SS wire in the second molar slot and first molar auxiliary tube, then bend the mesial end of the wire from the first molar end. Then, consolidate maxillary first and second molar by tight roping with a .012 ligature wire.
Adjustment of power chain after ‘3DKP’ placement
① 3 weeks passed in the state of D2
② 3 weeks passed in the state of new D2 = Total D4 after addition to old D2 state
③ 3 weeks passed in the state of new D2 = Total D6 after addition to old D4 state
④ After removing the old chain, hang a new D4 and observe the teeth movement for the following 3 to 6 weeks.
※ D2: ‘Double’ refers to hang the strong power chain called Double two on the arm of ‘3DKP’ after inserting it into ‘3DKP’ arm and turning around WAVE1.2 (Yellow power chain in the figure above). Since there are 2 ones, respectively on the left side and the right side, it is called Double two(D4=D2+D2, D6=D4+D2).
